You can also look up how many points you have on there from the DMV website also. It doesn't tell you what they are for, just the number currently.

Currently your driving record is listed as:
You have a valid Nebraska license.
You have no points assessed against your license/privilege to drive.
Points are calculated by citation date, not conviction date. If you have any outstanding traffic violation(s) that the court has not yet reported, they would not be included in the above displayed calculation.

No one answered the question after 14 posts.
You have to go down town to the DMV office. It's $3 if i recall and they willl print you off your record. Included will be the citation, date and number of points. If you have taken any driver's safety courses that will also be listed and the dated completed. Stop class and or those violations do not show up on your print out.
FYI. What prints out is not what the city has on you and the city won't release what it has on you. Also the city's records go back far further than your driving record.
For example the city can see ALL tickets you have had in lancaster county and what they orginally were if you convinced someone to change the charge. I found this out the hard way when trying to convince a city att. to change my ticket. He started rambling off my record including things that had been "dropped".

I'm pretty sure you have to mail in the form david
negative... i'm sure you can, but you can also go downtown sign/fill out the form pay right there and have them print it off for you. i've done this several times in the last couple of years to include a couple months ago for a job application.
i've never waited more than 10 mins and the ladies are pretty quick about processing your request as well.

it's not a DMV location you go to for tags.
It's an office in the state building just north of windstream i believe.

yup.. if you ask for a full-detail one it adds a lil extra info for no additional charge... if you are trying to get it for someone else (employer) and want to keep it as limited as possible just fill out the form and give em the money..
